BIMO inspection readiness checklist

FDA Bioresearch Monitoring (BIMO) inspections of clinical investigator sites, sponsors and CROs, and IRBs follow a pattern: most 483 observations come from a short list of requirements, and the list depends on what kind of organization you are. This checklist ranks the regulations FDA actually cited organizations like yours for over the last five fiscal years — work through it before FDA does. Print it, assign owners, and keep the evidence ready.

The mechanics, so nothing surprises you

During the inspection The FDA inspector shows credentials and a Form FDA 482 (notice of inspection). Expect the inspector to request any records related to the study’s conduct. Designate one point of contact; answer what is asked, accurately.
Inspection follow-up Minor deficiencies may be found and conveyed verbally, and may be corrected during the inspection. An inspection with no 483, or only minor observations, is usually classified NAI (No Action Indicated). More serious deficiencies are listed on the 483 at close-out, most significant first. A 483 is not a final FDA determination, but will be reviewed by senior FDA officials for classification.
Respond within 15 business days A complete written response within 15 business days is considered before FDA decides on further action. Address each observation with root cause, corrective action plan, and a date.
The classification An inspection with a 483 is usually classified VAI (Voluntary Action Indicated) or OAI (Official Action Indicated); FDA assigns the classification after review. OAI can lead to a warning letter — see Recent enforcement for how often, and how long after.
What this is (and isn’t): the ranking reflects how often FDA cited each regulation in its record (drug & biologic trials in this release) — a frequency signal, not legal advice or a complete audit program. Rare findings can still be serious. Pair this with your own quality system and, where stakes are high, qualified regulatory counsel.
Methodology. This checklist ranks the regulations FDA cited most often, in the publicly available Bioresearch Monitoring (BIMO) record, against organizations of the selected kind over the fiscal years named above; example observations are verbatim FDA 483 language from that record. It is a frequency signal, not legal advice or a complete audit program — rare findings can still be serious. Drug and biologic clinical research scope.

Clinical Precedent™ — clinical site intelligence from the FDA record Built from FDA Bioresearch Monitoring data. This release covers CDER and CBER inspections of clinical investigators, sponsors, sponsor-investigators, CROs and IRBs — drug and biologic clinical research. At this time, it does not cover bioequivalence (BEQ), Good Laboratory Practice (GLP), postmarketing adverse drug experience (PADE) or REMS inspections, device trials (CDRH) or veterinary products (CVM). The service reports the record; it does not make recommendations or provide regulatory advice. It does not cover other factors in clinical investigator selection — therapeutic-area expertise, site capabilities, your own history with the clinical investigator, enrollment track record, and your qualification visit.
